Community Grants Eligibility Criteria:
Grant requests should not exceed $2,500.
Requests include, but are not limited to: annual fundraisers, health fairs, and community education summits.
Event or Project must be held in Santa Clara County and/or Northern San Benito County.
Organizations are eligible to receive one (1) Community Grant per fiscal year.
Organizations that have an active Health Partnership (HP) grant will not be eligible to apply for a Community Grant until the completion of the HP grant. However, in the event that a HP grantee plans to have a special event during the grant period, HP grantees will be encouraged to build event support into its HP project budget.
Requests must be submitted at least 60 days prior to the event or project start date and can generally expect a response within 30 days.
The Health Trust encourages new applicants to apply for funding as we like to rotate grants among different agencies.
Requests will be considered based on the following criteria:
The extent to which the agency advances health equity in the community as evidenced by:
The agency’s proven capacity to advance health equity.
The agency’s demonstrated impact of advancing health equity.
The agency’s ability to connect with diverse audiences and establish strong partnerships.
For requests supporting community events e.g. health fairs: The number of participants who will benefit from the event and the measurable demonstrated benefit (e.g. during the health fair, we plan to provide 250 glucose and blood pressure screenings to older adults).
